Reportable Incident List (Incidents that Require a Critical Incident Report)

All 911 and Non-Emergency Calls:​	Any call made to 911 or the local non-emergency line reporting an incident and/or requesting emergency response.​
Medical Emergencies:​	Any incident where first aid is administered by staff and/or where staff activate the emergency medical system.​
Known or Suspected Overdose:​	Any medical emergency where staff know, or suspect, opiate overdose played a role.​
Injuries:​	Any incident resulting in an on-site injury of staff or participants.​
Self-Harm, Suicide, and Attempted Suicide:​	Any incident involving self-harm, attempted, or actualized suicide.
Communicable Disease Exposure:​	Any confirmed or suspected exposure to a communicable disease (e.g. Covid-19, flu, scabies, MRSA, etc.).​
Infection Control:​	Any measures taken by staff to reduce infection risk (e.g. site masking requirements, covid-19 mass testing, UV-C room sanitization, etc.).​
Biohazardous Accidents:​	Any incident where staff or participants are exposed to biohazardous materials (e.g. blood, vomit, feces, sharps, etc.).​
Violence:​	Any incident involving physical violence or the intent to harm others regardless of outcome.​
Sexual Assault:​	Any incident of unwanted conduct of a sexual nature that is inflicted upon a person or compelled by physical force, manipulation, threat, or intimidation.​
Possession or Use of a Weapon:​	Any incident where a weapon is used or is found in the possession of an individual.​
Possession or Use of Illicit Substances:​	Any incident where there is possession or use of intoxicants (including alcohol and cannabis) on or nearby a worksite.
Theft:​	Any incident where staff know or have reasonable suspicion to believe theft has occurred; regardless of resolution.​
Alarm Activation or Site Evacuation:​	Any incident where a building alarm is activated or where staff evacuate a site for any reason.​
Other Sentinel Events:​	Any important or notable events which are not described in the above categories.​
